Prekindergarten

North Shore offers Voluntary Pre-Kindergarten to our neighborhood children.  Pre-K students must be 4 years old on or before September 1, 2009

April 20, 2009 - June 7, 2009:  Monday - Friday/8:30 - 2:00 (Office closed June 8,9)
June 10, 2209 - August 29, 2009:Monday - Thursday/8:30 - 2:00

To register for the PreK program, you must provide:

1) Proof of address  (Mortgage/Rental agreement on official letterhead, Utility bill, Credit Card or Bank statement, etc. Any mail used as Proof of address must be dated within the previous 30 days.)
*  We are unable to accept cell phone bills, Driver's License, or mail older than 30 days.

2) Birth Certificate

3) Social Security Number (Social Security Card is requested)

4) Physical Form dated within the year (Yellow HRS form)

5) Current Shot Record (Blue form)

Kindergarten

To register for Kindergarten, you must provide:

2) Health Exam Form dated after August 8, 2008 (Yellow HRS form)

3) Current Shot Record (Blue form)/Current Shot record must show 2 Varicella (Chicken Pox immunizations)

4) Social Security Number (Social Security Card is requested)

Grade 1 - Grade 5 Registration

To register for grades 1 - 5, you must provide:

1) Proof of address  (Mortgage/Rental agreement on official letterhead, Utility bill, Credit Card or Bank statement, etc. Any mail used as Proof of address must be dated within the previous 30 days.)
*  We are unable to accept cell phone bills, Driver's License, or mail older than 30 days.

2) Copy of last report card is requested.

Please note:  There is no open enrollment for 6th and 7th grade students

Magnet Students

North Shore also offers Kindergarten - 7th Grade students outside of our attendance area through Duval County's Magnet Program.

Each student currently enrolled in a Duval County Public School in Grades Pre-K - 10 receives a preprinted application in the mail in early January. Additionally, applications are available at the Magnet Office, located in the School Board Administration Building on Prudential Drive and via the Magnet Office Website.

Beginning with the 2009-2010 School year,
students must enroll by their 5th Grade year.
