ACHS in conjunction with the St. Vincent’s Mobile Health Outreach Ministry will be offering free sports physicals to any current students or rising 9th graders on June 5th and June 6th, in the ACHS Auditorium. Male students can receive physicals on June 5th, from 8:30 am to 1:00 pm. Female students can receive physicals on June 6th, from 8:30 am to 1:00 pm. Students must bring with them 1) The FHSAA Physical Evaluation Form (EL2), and 2) The St. Vincent’s Consent Form.
