Be
prepared to turn in ALL Athletic Forms on the first day of
conditioning/tryouts. Athletes will NOT BE ABLE TO PARTICIPATE
without the following; HS Eligibility, Physical, Consent and
release, Compliance Affidavit, Copy of Insurance Card, Copy of Birth
Certificate.
