 Student Supplies Generic List

DOCUMENTS REQUIRED FOR REGISTRATION:

bulletBirth Certificate - student must be 5 years of age on or before September 1st to enroll in Kindergarten.
bulletImmunization Record
bulletPhysical - dated within 1 year of starting school.
bulletStudent's Social Security Number
bulletProof of Address - Bill or statement recently dated. Will also accept rental or buyer's agreement.
bulletProof of Grade Level - student's last report card
bulletIf you have any questions or for more information regarding registration please call the main office at 573-1100. All requirements must be met before registration will be accepted.

If you do not live in the Chimney Lakes Elementary attendance area, but do live in Duval County,you may apply for our magnet program. Chimney Lakes Elementary is a International Studies magnet school featuring high academic standards, student productions as well as ethnic dance, foods and arts. Applications may be obtained from the school but must be submitted to the Magnet Programs Office for acceptance. The Magnet Office must approve your application before you can register your child so applying early is recommended. OUT OF COUNTY MAGNET APPLICATIONS WILL NOT BE ACCEPTED.

CLE Homework Policy
When a student is out of school for one day, either for illness or family business, the student(s) can get the work the next day upon return to school.

When a student is out of school for either illness or family business for more than one day, a parent may request make up work prior to the students return to school by calling the teacher at school. Teachers have 24 hours to prepare the work and materials for parent pick up in the main office. Students have one day to turn make up work into the teacher for each day they are absent. 

Teachers are not to provide homework and/or classwork in advance of the absence.

Parents may not go to a classroom to request school or class work unless invited by the teacher.
