Extended Day
Payment Schedule for 2011-2012
We would like to welcome you and we hope you
will find the following information helpful as well as informative.
Early Dismissal days: We do not accept enrollment for
Early Dismissal days only. Student would need to be enrolled in the
regular after school Extended Day Program in order to stay on those days.
Extended Day Care hours
are 7:00AM until 8:30AM and 3:00PM until 6:00PM. This program is not a babysitting
service, but an extension of the regular school day, providing a continuous learning
experience for each student.

| Payment |
Payments
are due as scheduled. Payments MUST be made in
person by the parent to the Extended Day Director or Extended Day office during the
Extended Day hours. A late fee will be added to payment received
5 working
days after the due date. Cash payments must be for the exact
amount.

Programs approved by the Duval County School
Board are required to receive payment prior to services rendered.
A late fee will be added to payments received after the due date. When
a child is registered in the program, parents are given a payment schedule for the entire
year. The payment schedule is the official notice. The
schedule should be posted in a conspicuous place as a reminder. Continual problems
with late payments may result in your child not being allowed to remain in or return to
the Extended Day Program. |
| Checks |
Checks are acceptable as payment. Checks
should be made payable to the school providing the service. It is district policy
not to accept post-dated checks or to hold a check for any amount of time. If a
check is returned for non-payment, the EDP will refuse to accept checks for future
payments. Payment would then be made with a CASHIER'S check, money order or
cash. |
| Hours |
Children may not arrive earlier than 7:00AM and must
be picked-up no later than 6:00PM. Non-compliance with this policy will result in
dismissal from the program. |
| Warning |
Citizenship warnings will be given to children who
disobey instructors or Student Code of Conduct. |
| Property |
Children are responsible for their own belongings.
Names should be written on all belongings and lunch boxes. |
| Pick-Up |
Children will be allowed to leave with
persons who have written consent from parents only. We will not
accept notification by telephone. Designated persons must show identification. |
Extended Day Discipline Plan
Extended
Day Students are expected to:

Respect and obey their teachers at all times
Students will not argue or talk back to teachers
Students will do as they are told the first time they are told |

Follow the student code of conduct rules |
 |
Bring a nutritious snack and a bottle of
water |
 |
Leave anything that doesn't belong to them alone |
 |
Keep their hands to themselves |
 |
Clean up the classrooms before leaving at the end of the day |

Tell the truth at all times
Students who do not adhere to the discipline plan will not be allowed to attend the
extended day program. The first two times a student breaks the rules, a citizenship
warning will be sent home to be signed and returned by parents. The third time, the
students will be suspended for one week. The fourth time, the student will be
expelled from the extended day program for the remainder of the year.

Extended Day Parents are expected to:
Talk with their children about these expectations
Provide reinforcement of these rules
Conference with the extended day teacher if requested
Pick students up no later than 6:00PM
