Registration for School

REGISTRATION FOR KINDERGARTEN

Registration for most kindergarten and first-grade students occurs during the months of May through August at any Duval County public elementary school.  Under Florida Law, a child must have successfully completed Kindergarten in order to be eligible for first grade.  Children may enter kindergarten if they will be five years old on or before September 1 of the current year.  They may enter first grade if they will be six years old on or before September 1, on the current year, and have successfully completed kindergarten.  This is Florida law, and there are no exceptions.  Florida laws also require all children who will be six years old by February 1, of the current year, to attend school, and children must attend school until they reach the age of 16.

    When registering a child for school, parents must provide school personnel the following:

  1. Proof of child's birth date - Child must be five (5) years old on or before September 1, of the current year.

  2. Certified birth certificate (no copies please)

  3. Florida certification of immunization or an exemption

  4. School physical examination certificate (performed within one (1) year prior  to date of entry) (HRS form 3040 furnished by doctor's office) or other official certificate of a health examination that addresses the student's health  status.

  5.    Up-to-date immunization record (HRS Form 680)

  6.    A second MMR inoculation as well as Hepatitis series (HepB/HBV) are   required for students entering Kindergarten through fifth grade - also required, either the varicella (chicken pox) vaccine or documented history of varicella disease Pre-K through second grade.

  7.    A social security card or number.

  8. .  Proof of address - utility, telephone, cable bill, a lease agreement, or contract on a home, etc.    

     When a child has been enrolled in another school district, parents are asked to provide the latest report card from that district.
