Parents,

We would like to extend a warm to you from the staff of Don Brewer Elementary School. We are dedicated to providing an enjoyable yet enriching environment for the students in our care. We look forward to having an exciting year with you and your child.

   Definition

The Extended Day Program for students in Duval County Public Schools is defined as before and after school enrichment in the home school for students of working parents, grades K-5.

Eligibility

To be eligible for enrollment in the Extended Day Program, a child must be enrolled as a student in the school offering the program.

Days and Times of Operation

The Extended Day Program shall operate only on days that school is in session for students. The school day is broken up into two sessions, a morning session and an afternoon session. The morning session begins at 6:45 A.M. and the afternoon session will operate until 6:00 P.M.

The morning session of the Extended Day Program shall consist of informal, non-structured activities in order to facilitate entry of the children into the structured school day.  Activities of a more formal, structured nature shall be offered in the afternoon session.

 Enrollment Fees

Enrollment fees are assessed to provide revenues necessary to hire supervisory staff, counselors, purchase supplies, and to cover other operational costs.  Fees are to be paid on a monthly basis. Collection of fees is handled in accordance with established Duval County Public School Board Policies and regulations.  The program must be self-sufficient with all expenses incurred by operation of the program paid from fees generated by the program.  The program will be discontinued in schools where it is not self-sufficient.

    Morning Session                          52$

    Afternoon Session                      100$

    Morning and Afternoons           152$

Parents may enroll their children in more than one session.  There is a three day grace period for all payments.  All payments received after this grace period will be assessed a late fee of $10.

 Returned Checks

All accounts in which checks are returned for either insufficient funds or account closed will be required to pay in cash or money order for the remainder of the school year.  There will be no exceptions regarding this policy.  All returned checks will be also assessed a service charge of $20.  All checks should be made payable to the school your child receives services from.

 Arrival and Departure

Transportation of children to and from the Extended Day Program is the responsibility of the parent.  Children will only be released to authorized parents and/or designees.  All parents and designees are required to show identification before a student is released.  Students who are picked up after 6:00 P.M. WILL be assessed a late fee of $1 a minute beginning at 6:05.  This fee is due by the close of the next school day.  If three (3)violations occur, your child will be removed from the program.

 Pick-Up Cards/Procedures

Children are to be signed out by an authorized adult daily.  Each parent will be given an Emergency Pick-Up Card to be kept on file in the Extended Day office.  It is important to include all phone numbers and to keep them current. Please notify Extended Day Staff of any changes so that we may keep your information up to date. All designees must have a valid state issued form of identification.  No child will be released to any individual who does not have proper identification.

Messages

The Extended Day Supervisors will not be available for phone calls during the school day. If there is to be any change in plans for pick-up, a note from the parent must accompany the child to school and be presented to the supervisors at roll call.  Telephone calls to the school office or verbal messages from the child cannot be accepted for dismissal.

Discipline

Positive reinforcement principles will be used in dealing with children.  If problems persist and affect the safety and educational enrichment of other children in the program, the Extended Day Supervisors reserve the right to suspend and/or expel a child from the program.  Input from parents concerning problems at home that may affect a child’s behavior is welcomed.  Parents are encouraged to discuss matters that affect the enrichment atmosphere of the program. Citizenship warnings for disciplinary reasons will be given to any child who fails to follow established school and Extended Day Program rules. Students will be given two warnings or inappropriate behaviors.  If such behaviors continue, the following system of referrals will result.

          First Referral – parent signature

          Second Referral - parent signature

Third Referral - one week suspension

Fourth Referral - child will not be allowed to return to the program.  All refunds are at the discretion of the supervisors. Once a child has been expelled from the program, they may not return.

 Withdrawal

Regulations of withdrawal of children from the Extended Day Program:

·          The supervisor should be notified immediately within three days of  anticipated withdrawal.

·          Refunds will be given to participants upon withdrawal from the regular school day and the Extended Day Program.
