Florida Law requires all
students entering, attending, or transferring to the seventh grade
to have documented proof of the following immunizations:
1. Proof of address in your
name (i.e. light bill, phone bill, rental agreement, etc.) within
the past 30 days.
2. Report card or transcript
showing grade assigned and any standardized test scores.
3. Proof of immunizations:
A Tetanus-Diphtheria (TD)
booster given within the past five (5) years;
A Second dose of the
Measles-Mumps-Rubella (MMR) vaccine;
The Hepatitis B Vaccine (three
shots given over a 4-6 month period); Sixth grade students need to
start this process now. It takes 7 months to complete the
requirements.
4. Copy of birth certificate.
(Optional)
5. Copy of student's social
security card. (Optional)
Students new to Florida Public
Schools must also provide proof of physical exam within the last
twelve months.
