1. Current physical

2. EL3 Liability form signed

3. Copy of insurance card

4. New to EHS: copy of birth certificate

5. Sports contract signed

6. GPA- incoming 9th graders are O.K.

All others must have an overall 2.0 GPA

Participants will receive all forms from Coach Bateh
