New students enrolling Englewood will need to provide the following information:

  1. Proof of address located in the Englewood district in the parent/guardian's name, dated within the past 30 days. Acceptable documents include:  power bill, credit card bill, mortgage or agency rental agreement, a piece of mail with the yellow USPS forwarding address sticker.
     
  2. If you are transferring from any school outside of the Duval County Public School System, you must complete an enrollment packet was well as provide the following information:
    -Birth certificate
    -Immunization record (current)
    -Medical physical (within the last year)
    -Prior school withdrawal information
    -Documentation of special programs (if applicable)
     
  3. All students must reside with their parents unless court documents naming a legal guardian are provided.  Notarized letters will not be accepted.
     

Englewood High School is not able to  phone or fax requests
for any documents needed for registration.
