New students enrolling Englewood will need
to provide the following information:
- Proof of address located in the
Englewood district in the parent/guardian's name, dated within
the past 30 days. Acceptable documents include: power
bill, credit card bill, mortgage or agency rental agreement, a
piece of mail with the yellow USPS forwarding address sticker.
- If you are transferring from any school
outside of the Duval County Public School System, you must
complete an enrollment packet was well as provide the following
information:
-Birth certificate
-Immunization record (current)
-Medical physical (within the last year)
-Prior school withdrawal information
-Documentation of special programs (if applicable)
- All students must reside with their
parents unless court documents naming a legal guardian are
provided. Notarized letters will not be accepted.
Englewood High School is not
able to phone or fax requests
for any documents needed for registration.
