REGISTRATION  REQUIREMENTS

TO ENROLL NEW STUDENTS AT FLETCHER HIGH SCHOOL

     

1. Verification of address in the Fletcher District:

Welcome to the Beaches!  We are pleased that you have chosen to enroll in Fletcher High School and look forward to serving you and providing your children a quality educational experience.
Please be aware however, that the parents of all students new to Fletcher High School are required to provide the following documentation to the school at the time of the child's registration.

Prior to admission into Fletcher High School, students must provide proof of residency within the Duval County School Board approved boundaries. A student can have only one address for purposes of establishing residency. A student shall be considered to have complied with residency verification requirements when parents/legal guardians have provided the following original (no copies) documentation.

    

 Four (4) documents are required to establish residency (one from each category below).  Only street addresses meet residency requirements. (Post office boxes are not accepted.)

    

      Category 1. A Parent/Legal Guardian’s Picture ID from the following list:
           a) Current Florida State Driver’s License or
b) Current Florida State ID Card or
                   c) Valid Passport or Consulate Issued Picture ID
showing street address

                      

Category 2. One of the following ORIGINAL documents with a parent/legal guardian’s name and address:
a) Current valid vehicle registration
                          b) Federal Tax Return filed within the past 12 months
                with W-2 forms attached (business returns
                   do not meet residency address requirements)

    

Category 3. One of the following ORIGINAL documents with a parent/legal guardian’s name and address:
a) Property Tax Bill indicating homestead exemption
     b) Rental or lease agreement that includes the property
manager’s name and phone number   
c) Home purchase closing statement

                         

        Category 4. A utility bill that indicates service with a parent or legal guardian’s name and address:
a) Electric bill                  
    b) Cable TV bill                   
c) Phone bill (not cellular)

If you do not have any of the documents listed above, bring whatever you have showing your name and address. If necessary, we will have you meet with one of the administrative staff. A home visit may be scheduled.
BE ADVISED THAT FRAUDULENT DOCUMENTATION USED TO EVADE RESIDENCY VERIFICATION IS A VIOLATION OF STATE LAW PUNISHABLE BY UP TO FIVE YEARS IN PRISON AND/OR A $5,000 FINE.

    

2. Immunization Record

  Florida HRS Immunization Guidelines state that students transferring from another state must present a Florida Immunization Certificate (HRS680) at the time of registration.  All students entering or repeating the 9th grade MUST MEET THE NEW IMMUNIZATION REQUIREMENTS. The new requirements are tetanus-diphtheria (Td) booster given within the past five (5) years; a second dose of measles vaccine (preferably MMR); and the Hepatitis B vaccine (a series of three (3) doses).

    

3. Physical

 All students entering a Florida school for the first time MUST have a current (dated within one year) school physical within 30 days of registration.

    

4. Birth certificate
(If new to Duval County, or not previously entered into the DCPS computer system.)

 Original or certified copy --  a photocopy will be made at FHS and the original will be returned to the parent.

    

5. Latest report card and transcripts.

    

6.  Exceptional Education IEP
(for ESE students only)

    

7. Social Security Card/Number
(recommended)

    

8. The following forms completed legibly:

a) Student Information Enrollment Form

b) Request for Records

c) Student Health Questionnaire

d) Emergency Card

e) Duval County Home Language Survey
(If "YES" is checked, contact ESOL @ 390-2202 and notify the counselor.)
