Day Fees (20 Day Sessions)

Purpose:
The
purpose of our EDP is to provide responsible supervision
of children of working parents living in the Garden City
attendance area. Our EDP will provide a safe and caring
environment in which each child may develop his/her
physical and intellectual potential at his/her own rate
of speed.
Rules:
1.
Students in the Extended Day Program must follow
the Duval County Student Code of Conduct.
2.
Students must respect other people and property.
3.
Students must not bring any dangerous items to
school.
4.
SAFETY FIRST! Fighting, throwing objects,
horseplay, or hitting other students or instructors will
result in immediate disciplinary action.
5.
Always report all injuries, no matter how small,
immediately.
6.
Students must report for roll call immediately
after school and remain with their instructors at all
times.
Warnings:
Citizenship warnings will be given to children who do
not follow the rules of Extended Day or the Duval County
Code of Student Conduct. First and second warnings must
be signed by parents and returned the next day.
Third warnings will be signed and returned and will
result in a one week suspension from the
program. When a child receives a fourth warning, a
conference with the parent and the director will be
required before the child may return to the program.
The fifth warning will result in the student being
expelled from the program. IF EXPELLED, NO REFUND
WILL BE GIVEN FOR THE BALANCE OF THE SESSION. An
immediate suspension could result if a child were to
injure another person while participating in the
Extended Day Program.
Pick-up and Drop-off Procedures:
Transportation of children to and from the Extended Day
Program is the responsibility of the parents.
Children will not be allowed to leave with anyone except
the parent and/or the person designated by the parent.
A note to the Extended Day Director is requested for
special pick-up. Verbal messages cannot be accepted
by the Director from the student.
Arrival and Departure:
Students may not arrive before 7:00 am and must be
picked up by 6:00 pm.
If a
child is picked up late, there will be a fee imposed.
The fee will be $1.00 per minute after 6:00 pm. This
fee is due by the close of the next day. If three
violations occur or the fee is not paid, the child will
be removed from the program. The school clock is used
when determining late fees.
Withdrawal from EDP:
If, at
any time, it becomes necessary to withdraw a child from
the Extended Day Program, a note should be sent to the
Director. A child will not be withdrawn until a note
has been received.
Extended Day Payment Policies:
Payments are due as scheduled. All payments should be
dropped in the Extended Day payment box. All payments
should be in an envelope. Cash payments must be for the
exact amount. No change will be given.
Checks
will be accepted. All checks must include a name,
address, home phone, driver’s license number, and a work
number. It is district policy not to accept post-dated
checks or to hold a check. If a check is returned for
non-payment, there will be a charge of $40.00 and no
other checks will be accepted.
Payment is due in 4 week sessions (20 school days).
There is no charge for school holidays. District policy
states that a student may not remain in the Extended Day
Program unless their account balances are paid on a
current basis. If payment is not made prior to
the 1st day of the session, the child will
not be admitted to the Extended Day Program until the
fee is paid in full.
Please
keep your receipts.
They will serve as documentation for tax purposes. The
Federal Tax Identification number is 59-6000-589.
A
$10.00 late fee will be assessed if payment is not made
after five working days from the payment due date.
Personal Belongings:
All
student belonging should be clearly marked with the
child’s name. The child is responsible for all
belongings and the Extended Day Program cannot reimburse
anyone for lost items. Children may not bring their own
toys to Extended Day.
Health:
Children who are ill should not be brought to school or
to Extended Day. If a child becomes ill, parents will
be notified and the child will need to be picked up. |
