Sick Leave Buy-Back

If you would like to receive payment in lieu of unused accumulated sick time, please read the buy back memo and fill out the sick leave buy back form.  All requests for payment must be turned in to the Payroll department by the dates on the memo.

IMPORTANT INFORMATION FOR ADMINISTRATORS

If you have, or are approaching 62.5 days of leave time saved, please read this important memo concerning a potential loss of time.

Need Another W-2?

Fill out the Request form (word) (PDF) and return it to payroll.

In order to insure the privacy of the employee, an original signature is required on this form; a faxed copy will not be accepted to release the W-2 form. A faxed copy can be used to start the processing of a duplicate W-2, but the original form will be required to pick up the W-2. Duplicate W-2 forms will only be released to the person whose name appears on the W-2 form.  Payment of the $5.00 service charge will also be required to release the W2.  Payment should be in the form of cash or money order made payable to Duval County Public Schools.  Please allow 48 hours upon receipt by the Payroll Department to process this request.
