opportunity scholarship program
This State of Florida program
allows parents and guardians whose students were
enrolled in 2008-2009 and/or assigned for 2009-2010 to a
failing school to attend a higher-performing school. A
failing school is one that has received a failing grade
twice within a four-year period.
Procedures:
If a school is designated as failing, the student’s
parent or guardian is notified by mail. If the parent
chooses to participate in the Opportunity Scholarship
Program, the parent must submit an application. The
parent may choose one of the following options:

Parent or guardian may choose to have their
student remain in the current or projected
school.

Transfer the student to another public school
that has been graded ‘C’ or higher. This year, the
School Choice Office is offering a choice of three
K-5, two middle and 10 high schools. The parent
should make a first and second choice on the
application.
However, you may be assigned to any of the school choice options
listed for your student’s grade level.
The parent must fill out
an Opportunity Scholarship Program Student Transfer
Application and either mail or bring it to the
School Choice Office in the Duval County Public
Schools’ administration building at 1701 Prudential
Drive, 32207, by
July 31, 2009.
DO NOT FAX THE APPLICATION. You will be notified by mail regarding
the 2009-2010 school assignment for your child.
If a parent or guardian
fails to receive a pre-printed Opportunity
Scholarship Program Student Transfer Application in
the mail and would like to apply, download an
application from duvalchoice.com and return it along
with two forms of proof of address to the School
Choice Office.
- Transfer the
student to another public school that has been
graded ‘C’ or higher that is NOT in Duval County. It
is the parent’s responsibility to contact the other
school district to determine if that district will
accept the student based on that district’s adopted
criteria. If accepted by the other school district,
the parent must contact the School Choice Office at
390-2144 to obtain a release from Duval County.
Transportation to the other school district will be
the parent’s responsibility.
