hospital homebound
The goal of the Hospital/Homebound Program is to help students maintain their educational performance while confined at home or the hospital. Working closely with family members, the Northeast Florida medical community, and home schools, school district teachers provide quality instruction in flexible ways to meet the individual students’ needs. The instructional program is delivered to students in these ways:
- Home or hospital by a visiting teacher
- Bi-school enrollment for students who can attend school on a part-time basis due to a significant medical or psychiatric condition
- A virtual or web-based program for middle and high school students
Services can be requested by completing the
application (see link above) or by calling the Hospital/
Homebound Program office at 904-348-7849. After the form
is completed by a parent and physician, it must be
mailed, or faxed (904-348- 7757) or delivered to:
