hospital homebound
The goal of the Hospital/Homebound Program is to help students maintain their educational performance while confined at home or the hospital. Working closely with family members, the Northeast Florida medical community, and home schools, school district teachers provide quality instruction in flexible ways to meet the individual students’ needs. The instructional program is delivered to students in these ways:
- Home or hospital by a visiting teacher
- A telecommunications system, in which students receive instruction over the telephone within a virtual classroom
- Bi-school enrollment for students who can attend school only part time due to physical or mental illnesses
Services can be requested with a medical form that is available at all schools or by calling the Hospital/ Homebound Program office at 904-348-7849. After the form is completed by a parent and physician, it must be mailed or delivered to:
