Each year, the Florida Department of Education invites parents of students with an Individualized Education Plan (IEP) to complete a survey. This survey helps evaluate how effectively schools are partnering with families and support meaningful parent involvement in their child’s education.

We encourage all parents, guardians, and adult age students to participate and share their feedback. The ESE Parent Survey is available in 10 languages and can be completed electronically or in paper format, based on your preference.

The survey is open to all parents and/or guardians of students receiving Exceptional Student Education (ESE) services from preschool through grade 12, as well as students with disabilities who are 18 years of age or older. It is not intended for parents of students receiving only gifted services through an educational plan. The survey features straight forward questions and takes two to five minutes to complete.

Parent registration is required each school year for all students— including those receiving special transportation— in order to be assigned a bus for the upcoming school year.  

Transportation will not be scheduled until parent registration is completed.

Please follow the steps below to register:   Log in to your parent/guardian Focus account and select your child’s name.  Select Child Info Child Info.   Select Forms.   Select 2026–27 Transportation Registration Form 2026–27 Transportation Registration Form.   Review your address. The primary address primary address determines your child’s bus assignment.   If the address is correct: Go to “I am requesting bus stop transportation for my student I am requesting bus stop transportation for my student” and select Yes   If the child is served in a Self-contained ESE Program Self-contained ESE Program: Complete the 3rd section as appropriate.   Click Submit Request Submit Request at the bottom of the form in blue.  

If you choose not to access transportation for the school year, no action is required

The infographic includes the following information: Some students may be eligible for an IEP, or Individualized Education Plan. An IEP is a written plan created to support a student’s unique learning needs. It is important to know that having a medical diagnosis (such as ADHD, autism, or anxiety) does not automatically mean a student will need an IEP or be eligible for special education services. Schools look at how a student’s needs impact their learning and whether specialized support is required in the school setting. Not every student with a disability is eligible for an IEP, but the information to the right will help you understand what an IEP is and how the process works.
