Adult Role Model

Thank you for your expressed interest in the 5000 Role Models of Excellence Project (Duval Chapter). This program seeks to match minority male students with images of successful minority men from various career fields. We have included two links for you to register to become a Role Model which will put you that much closer to reaching your goal of speaking to our students. Our program is located in elementary, middle, and high schools through Duval County Public Schools. We have a simple two-step role model registration process:
Student Role Model

The 5000 Role Models of Excellence Project aims to positively impact the lives of young men by providing positive role models, meaningful experiences, and opportunities that empower them to make good choices, practice healthy living, demonstrate responsibility and respect, and achieve their goals.
A prospective student member should be a male student (membership is not exclusive to minority males) enrolled at a participating 5000 Role Model of Excellence Project school. The 5000 Role Models of Excellence Project is a uniformed program. Parents/guardians are responsible for providing the required uniformed, which includes:
White long-sleeved dress shirt
Black dress pants
Black dress shoes
Black socks
Black belt
Student members are expected to attend weekly meetings and participate in program events while wearing the required uniformed. Students must complete the online application and submit any additional required documentation to the Site Directors at their school.
Eligibility Requirements:
To be considered eligible for membership, a student must meet at least two of the established qualifying criteria.
Registration Process
Step 1: Complete the Student Role Model Application
Register to become a student Role Model by completing the online application by clicking Become a Student Role Model!
Step 2: Complete the Site Enrollment Process
After submitting the application, the student will be contacted by the Site Director with information regarding a site meeting and completing any additional enrollment requirements.
