All student-athletes must complete the following before participating in tryouts or practice:

☐ Create or update your Aktivate account (Aktivate.com)

☐ EL2 Sports Physical (valid for 365 days) Form

☐ ECG Screening

• Incoming 9th graders

• Any 10th-12th grader who has never participated in FHSAA athletics
  (Required beginning with the 2026-2027 school year.)

• Submit ME1 or EL1 documentation in Aktivate.

☐ EL3 Consent and Release Forms (5 pages)

☐ Proof of Health Insurance

☐ Birth Certificate

• One copy on file (only required once)

☐ GA4 Affidavit of Compliance
(Required for homeschool, private school, charter school, and transfer students who did not enroll at Englewood as a freshman.)

☐ DCPS High School Sportsmanship Contract

☐ Complete NFHS Learn Courses
Upload certificates (4) to your Aktivate account after completing:

• Concussion in Sports

• Sudden Cardiac Arrest

• Heat Illness Prevention

• Sportsmanship

☐ Complete all required forms and upload them to your Aktivate account. Submit any documents that cannot be uploaded directly to the Athletic Director.
