Required Documents

**New - Starting July 1, 2026 All incoming 9th graders, new to athletics regardless of grade, or athletes from out of state must have an ECG screening before trying out for any sport in the State of Florida.

All Athletes must have required documents before tryouts.

• EL 1 (ECG Screening Form) (upload to Aktivate) for incoming 9th graders, new to sport, or out of State athletes.

• EL 2 (physical) (upload to Aktivate)

• Birth Certificate (upload to Aktivate)
