The Florida Department of Health-Duval County will be hosting a Health Fair on Friday, April 11th, from 10am – 2pm at Central Health Plaza, 515 W. 6th Street, Jacksonville, FL 32206.
The Immunization Clinic will be offering the following vaccines for students: TDaP vaccines (required for 7th grade entry), Menquadfi (meningitis vaccine), Gardasil (HPV vaccine), MMRV (Measles/Mumps/Rubella/Varicella vaccine) and flu vaccines only.
No appointment is necessary, but a parent or guardian must be present to authorize consent for vaccines. An updated Florida Certification of Immunization (680) Form will be provided for students receiving the TDaP vaccine.
Please note, no other immunization services, such as additional school vaccines or translation/transfer of shot records, will be provided at this event.
