The Florida Department of Health-Duval County will be hosting a Health Fair on Friday, April 11th, from 10am – 2pm at Central Health Plaza, 515 W. 6th Street, Jacksonville, FL 32206.
o The Immunization Clinic will be offering the following vaccines for students: TDaP vaccines (required for 7th grade entry), Menquadfi (meningitis vaccine), Gardasil (HPV vaccine), MMRV (Measles/Mumps/Rubella/Varicella vaccine) and flu vaccines only.
o No appointment is necessary, but a parent or guardian must be present to authorize consent for vaccines. An updated Florida Certification of Immunization (680) Form will be provided for students receiving the TDaP vaccine.
o Please note, no other immunization services, such as additional school vaccines or translation/transfer of shot records, will be provided at this event.
o Please contact School Health at 904-253-1580 if you have any questions.
