Identify whether the student is new to DCPS or returning to DCPS
For registration you need students:
Birth Certificate,
Florida Immunzation Records,
Physical Exam,
Recent Report Card, and
IEP/504 if applicable
You also need proof of residence
Two documents listed in column A or One Document in Column A and two from column B
Column A
JEA Bill or TECO bill
Mortgage agreement
Lease agreement
Drivers License or State ID
Column B
Automobile Insurance
Credit or Bank statement
Payroll statement
Phone bill
Vehicle Registration
