Below is the link to Athletic Clearance and Preparticipation Physical Evaluation form required to be completed by the Florida High School Athletic Association to participate in athletics. These forms must be completed every year a student participates in athletics. The completed forms must be completed before the first practice. A student may not participate in athletics until all completed forms are completed.

Beginning July 1, all incoming freshmen and any high school student who has not yet participated in athletics will be required to complete an ECG screening prior to participation.

To clarify, for current high school students, trying out for a sport counts as participation—even if you did not make the final roster.

Please note that without a completed ECG screening, students will not be allowed to participate in athletics.

All required documentation will continue to be uploaded through Aktivate (link is below), and there is now a designated section on the platform for ECG information.

We will have a clinic for ECGs on June 10th from 9:00am - 12:00pm in the cafeteria. Pre-registration is required! Please register here to participate. When you register, if you choose financial assistance, the $20 fee will be waived.
